How do I send you my prescription?

In order to process your order, we need the following information:

• • Doctor's name, signature, and office phone number
  • Patient's Name 
  • Date the prescription was dispensed and date it expires.

If you need to renew your prescription, you can take our online vision test from the comfort of your home. The test takes about 5 minutes to complete and a licensed doctor will send you a renewed prescription within 24 hours. You can get started here!
